چکیده

INTRODUCTION The Segond injury corresponds to a marginal avulsion fracture of lateral tibial plateau, resultant from flexion traumas, stress in varus and internal rotation. The structure responsible for avulsion is the lateral collateral ligament, and anterior cruciate ligament and lateral meniscal injuries are also frequently present(1-4). Hall and Hochman(5) provided the first report of an injury described as “Medial Segond-Type Fracture”, where clinical and imaging findings would be opposite to those of the classical Segond injury. They reported an avulsion fracture of the medial tibial plateau, caused by medial collateral ligament insertion (deep portion), combined to posterior cruciate ligament rupture and medial meniscal injury. Escobedo et al.(6) described three additional cases of this unusual injury, naming it as “Reverse-Segond Fracture”, these being the only reports in literature. In this article, we describe a case similar to the four previously published articles, where an avulsion fracture of the medial plateau was reported caused by deep medial collateral ligament and posterior cruciate ligament injury. This patient presented with concurrent anterior cruciate ligament injury, similarly to one of the cases previously described, and, differently from the others, it didn’t show meniscal injury. Reverse Segond Fracture Associated with Anteromedial Tibial Rim and Tibial Attachment of Anterior Cruciate Ligament Avulsion Fractures

Reverse Segond fracture is an uncommon avulsion fracture of the tibial attachment of the deep portion of the medial collateral ligament of the knee. We report a reverse Segond fracture associated with anterior cruciate ligament tibial avulsion fracture and anteromedial tibial rim fracture. Unlike previous reports, the combination of reverse Segond fracture, anteromedial tibial rim fracture, and...

Medial and lateral segond fractures in a skeletally immature patient: a radiographic marker for the multiply injured knee.

Marginal fractures of the medial tibial plateau have been reported in the literature as a secondary type of Segond fracture. Some reports described this entity in the setting of combined injuries such as root avulsions of the medial meniscus, complete disruption of the posterior cruciate ligament (PCL), partial tear of the anterior cruciate ligament (ACL), and tears of the medial meniscus and m...

Reverse Segond fracture without PCL injury

This case involves a unique reverse Segond fracture pattern with ACL and MCL tears that does not have the usually involved PCL tear or obvious meniscal tear. It further shows the importance of recognition of the medial tibial avulsion fracture as an indicator of more severe internal derangement and instability of the knee. Early recognition prompts a more thorough evaluation and management of i...

Absolute Isolated Avulsion of the Anterolateral Complex of the Knee: A Segond Fracture

The Segond fracture is defined as an avulsion of the proximal anterolateral tibial plateau region originally described by the French surgeon Paul Ferdinand Segond in 1879 [1,2]. This injury is widely accepted as a pathognomonic radiographic marker of anterior cruciate ligament (ACL) injury [1-3]. However, some reports describe this injury in the presence of an intact ACL in skeletally immature ...

Adolescent Segond fracture with an intact anterior cruciate ligament.

Segond fractures, or avulsion fractures of the proximal lateral tibial plateau, have been well documented and studied since their original description in 1878. Segond fractures have a widely recognized pathognomonic association with anterior cruciate ligament (ACL) injuries and often prompt orthopedic surgeons to consider reconstruction following radiographic and clinical evaluation. Adolescent...
